The JavaScript and CSS should be placed on every page of your site, regardless of whether the chat button is presented, to ensure customers can continue the conversation even if they navigate away from the page they started on (i.e. moving from the contact page into product pages whilst in the chat). The chat window will not be able to run on any pages where the JS isn’t present.

The HTML snippet only needs to be placed on pages where you want to present a ‘start chat' button.
